HIPAA Notice of Privacy Practices

I. Purpose of this Notice

This notice describes how protected health information may be used and disclosed and how you can obtain access to that information.

II. Duty to Safeguard PHI

Inner Healing Resolutions, LLC is required to maintain the privacy of protected health information and provide notice of privacy practices. The practice may update its policies as permitted by law and will make revised notices available.

III. Uses and Disclosures

PHI may be used or disclosed for treatment, payment, and health-care operations as permitted by law. Certain disclosures may also be required or permitted for emergency treatment, public-health activities, health oversight, workers’ compensation, legal proceedings, research, appointment reminders, and other legally authorized purposes.

Disclosures outside permitted categories generally require written authorization. An authorization may later be revoked in writing, subject to actions already taken in reliance on it.

IV. Your Rights

Patients have rights concerning access to PHI, requesting copies, requesting restrictions, requesting confidential communications, and obtaining an accounting of certain disclosures. Applicable timelines, fees, exceptions, and review rights are governed by law.

V. Complaints

You may raise concerns about privacy practices with the practice or with the U.S. Department of Health and Human Services. Retaliation for filing a privacy complaint is prohibited.

VII–X. Breach Notification, Access, and Related Rights

The notice addresses breach notification, treatment of PHI after death, restrictions on certain disclosures to health plans, electronic access to PHI, and authorization requirements for psychotherapy notes, marketing disclosures, and sale of PHI.

XI. Effective Date

The public notice states an effective date of September 1, 2024.
